​​​​Requesting a certificate of professional conduct 

You may be required to submit a certificate of professional conduct (CPC) when applying for hospital privileges in BC. This is an official document that confirms your current licensure held with BCCNM (and the legacy college, the College of Midwives of British Columbia). Please note: a CPC cannot be issued until you are granted licensure. 

How to apply

  1. ​Sign into your account and select Request Certificate of Professional Conduct

  2.  Complete the online form and submit the required payment of $60.00 (plus GST). An email address can be included in the address line. 

  3. Once we receive your request and payment, we will create an official document and will email it to the privileging department indicated. It may take up to four weeks for your request to be processed.​

  4. You will receive an email confirmation once the CPC has been sent.

The certificate of professional conduct includes:

  • BCCNM license number, certificate name, current licensure status and effective date
  • Date first licensed
  • Current practice name and address on the registry
  • Any certifications you may hold
  • Any current restrictions, limits or conditions on your license
  • Statement regarding whether or not your licensure is in good standing
  • Statement regarding whether or not an unresolved written complaint has been received by BCCNM or your licence is under investigation

​​​​Requesting a letter of standing and professional conduct 

You will be required to provide a letter of standing and professional conduct when you apply to another regulatory body or educational institution. ​This is an official document that shows your current licensure status and licensure history with BCCNM (and the legacy college, the College of Midwives of British Columbia). 

Important: Letters of standing and professional conduct are sent directly from BCCNM to the organization you are applying to. 

How to apply

  1. ​Complete the Request for Letter of Standing and Professional Conduct: Authorization and Consent for the Release of the Information and submit to registermidwives@bccnm.ca​

  2. Pay the fee​. Instructions will be sent to you after your app​​lication has been received. 

  3. ​Once we've received your request and payment, we will create an official document and will mail it to the regulatory body or education institution indicated on your request form. It may take up to four​ weeks for your request to be processed.​

  4. You will receive an email confirmation once the letter of standing and professional conduct has been sent.

The letter of standing and professional conduct includes:

  • BCCNM license number, initial licensure dates (including provisional registration, if any) and current licensure status

  • Date of birth and former name(s) if any

  • Any current restrictions, limits or conditions on your license

  • Compliance with BCCNM’s Quality Assurance Program

  • Statement regarding whether or not your licensure is and has been in good standing

  • Statement regarding whether or not an unresolved written complaint has been received by BCCNM or your licensure is under investigation​
